Job Description

Full Time ---- Salary Range: $23.53 - $28.85

Organizational Responsibilities:

  • Adhere to, support and promote St. Francis Health Services’ Mission and Core Values, philosophy, goals and objectives while conducting company business.
  • Maintain confidentiality of company and individual receiving services related information at all times.
  • Serve on, participate in, and attend various committees of the care center as appointed by the Social Service Director.
  • Involve individual receiving services and their responsible person whenever possible in the admission process. The mentally competent individual receiving services will be involved in the signing of all important documents. Interpret the Care Center contract, policies, Bill of Rights, PSDA, Grievance Policy, and other pertinent information.
  • Provide individual receiving services advocacy and assist in resolving grievances.
  • Coordinate interdisciplinary individual receiving services discharge planning as delegated.
  • Provide additional support with administrative duties

Individual Receiving Services Care Responsibilities:

  • Does not disclose individual receiving services’ protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Ensure that all care is provided in privacy, as appropriate, and to knock before entering a individual receiving services’ room or individual receiving services care area such as tub rooms.
  • Report all grievances and complaints made by the individual receiving services, family, or visitors to the appropriate supervisor following the care center’s established procedures.
  • Report to licensed staff any necessary observations made regarding potential changes in individual receiving services status, utilizing the STOP AND WATCH tool.
  • Coordinate family and applicant tours, explain costs and services available, and assist in completing application forms as delegated.
  • As delegated, complete the Psychosocial Assessment, assessment of Vulnerable Adult Status, assessment of Discharge planning goals, and begin the interim plan of care.
  • Maintain documentation of services in accordance with care center policies.
  • Coordinate admissions, room transfers, discharges from the care center and other related social services tasks as delegated.

 Individual Responsibilities:

  • Report all complaints and grievances, according to company policies and procedures, which are made by the individual receiving services, family, or visitors and make a written/oral report to supervisor indicating what action(s) were taken to resolve the complaint or grievance.
  • Report any communicable or infectious disease that you contract to the appropriate supervisor.
  • Managing correspondence, handling emails, phone calls, and other forms of communication to maintain clear and organized communication.
  • Organizing files and documents, keeping files and documents organized and accessible for quick reference.
  • Scheduling appointments, managing and organizing schedules to ensure efficient use of time and resources

 Regulatory Responsibilities:

  • Understand and follow the Vulnerable Adult and Abuse Policy and reporting procedure.
  • Understand and adhere to Individual receiving services Bill of Rights.
