Job Description

Minimum Qualifications:

Education: Bachelor’s degree from an accredited college or university in any field.

Experience: Prefer one year work experience providing at least twelve months of related case management services to children and families. Must be able to assist families struggling with multiple stressors. Understanding of the special problems and treatment of consumers with mental illness and child welfare issues is essential. Understanding of community resources and social support development is required.  

Must be 21 years of age.

Negative Drug Screening prior to hire and throughout employment.

Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver’s License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).

Working Conditions: Requires both indoor and outdoor work with outreach to off-site locations. Exposure to extreme heat and humidity during the summer months. Schedule may be irregular, based on client availability. Work is primarily off site in client family homes and community locations.

Computer Literate: Must be able to work in an electronic health record.

Job Duties:

  • Provides services that are consistent with the program methodology and in conformance with all program standards.
  • Works to make initial appointment as soon as possible after receiving a referral for services. Meets with family as dictated by level of need. Establishes and maintains a trusting relationship with families at high risk for child maltreatment. 
  • Accepts and assesses the level of need of families referred for case management services. Meets with individuals and families to assess current needs. Assures that all program criteria are met before opening the individual or family.
  • Provide outreach with various community organizations to develop a relationship with each agency to ensure a smooth referral flow. 
  • Makes a minimum of once monthly face-to-face contact with all individuals and families or documents reasons family was not able to meet. 
  • Meets regularly with families to demonstrate and teach/model effective parenting skills in the home as needed. Teaches daily living skills such as budgeting, proper nutrition, and meal preparation as needed. 
  • Assesses and monitors the individual/family’s progress and completes addendums to the Assessment and Service Plan as needed.
  • Acts as an advocate for the individual/family at all relevant staff meetings, school IEP meetings, court, etc or arranges for a substitute.
  • Links the individual/family with necessary resources to assist the family with needs and monitors the effectiveness of these interventions.
  • Works to contact the individual/family within 24 hours of a missed appointment.
  • Provides case management services that are culturally competent, solution focused, strength based, recovery oriented, and co-occurring capable as they are needed by program families.
  • Completes all documentation in timely manner including case records, progress notes, and outcome measures for Juvenile Welfare Board and any other program funders.
  • Uses appropriate crisis intervention skills and anticipates and prevents crises when possible.
  • Remains current in the general knowledge of psychotropic medication, side effects and DSM diagnostic criteria and discusses any concerns with supervisor.
  • Consults with program manager and assigned clinician(s) regarding cases.
  • Reports all risks to supervisor and ensures that risk notes and safety plans are completed as appropriate.
  • Meets regularly with program manager to review cases and job performance.
  • Keeps accurate records of caseloads and ensures that any required measures are completed and reported to data entry in a timely manner.
  • Meets required productivity standards.
  • Assists with quarterly and annual reports for the Agency as needed. 
  • Accomplishes individual goals.
