Job Description

This position is posted by Jobgether on behalf of a partner company. We are currently looking for a Care Connector in the United States.

This role is centered on providing compassionate, high-quality support to patients and members navigating complex healthcare and behavioral health systems. You will serve as a key point of contact, helping individuals understand available services, resolve concerns, and access appropriate care and community resources. Working in a remote-first environment, you will combine clinical awareness, strong communication skills, and problem-solving abilities to support members facing medical, behavioral, or social challenges. The role involves assessing needs, managing escalations including crisis situations, and ensuring timely coordination with internal teams and external providers. You will also help address social determinants of health by connecting members to relevant support systems. This position is both highly interpersonal and mission-driven, with a strong focus on advocacy, empathy, and care continuity.

\nAccountabilities
  • Serve as the primary point of contact for patients, members, providers, and behavioral health support units
  • Assess clinical and non-clinical concerns to ensure accurate information and timely resolution
  • Manage and triage member concerns, including crisis situations, and coordinate appropriate next steps
  • Support members in addressing social determinants of health by identifying needs and available resources
  • Conduct motivational interviewing and needs assessments to help members determine appropriate care pathways
  • Coordinate with internal departments and external providers to resolve eligibility, care, or service issues
  • Document and track member interactions, compiling data for reporting and operational insights
  • Educate members on available services, benefits, and community resources based on their coverage
  • Prepare and send formal communications, including letters and follow-ups regarding case resolution
  • Provide peer support, training assistance, and guidance on policies and procedures within the team
  • Perform outbound outreach calls to resolve complex eligibility or access-related concerns
  • Navigate multiple systems and tools to ensure accurate, efficient service delivery

Requirements

  • Bachelor’s degree in Social Work, Psychology, Public Health, Healthcare Administration, Human Services, or related field
  • Minimum of 1 year of relevant experience in healthcare, behavioral health, case management, or member support services
  • Strong communication and interpersonal skills with a compassionate, patient-centered approach
  • Ability to assess complex situations and apply critical thinking to determine appropriate solutions
  • Experience working with vulnerable populations or individuals facing medical, behavioral, or social challenges
  • Strong organizational skills with the ability to manage multiple cases and priorities effectively
  • Comfortable working in a fast-paced, remote environment with multiple systems and tools
  • Ability to manage crisis situations calmly and professionally
  • Strong documentation and data entry skills with attention to accuracy and detail
  • Knowledge of social determinants of health and community resources is highly desirable
