   Job Coach

Today, we are a leading not-for-profit agency employing 1200+ staff and providing over 70 program sites in Nassau and Suffolk. ACLD’s Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment.

In this position, you will:

  • Provide on-site job coaching for designated caseload
  • Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation.
  • Conduct observational assessments on site in order to assist the person supported in learning the tasks of the job
  • Travel train on the public transportation system and/or roadways
  • Maintain case files for each person supported.       
  •  Prepare all documentation for ACCES and OPWDD on a monthly basis
  • Act as a liaison between the person supported, family and employer
  • Attend all staff meetings, Life Plans, conferences and annual training requirements
  • Provide overall assistance as assigned

We require:

  • High School Diploma or equivalent; Associate’s Degree in Psychology, Sociology, Education or related field preferred.
  • Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred
  • Complete (within first year of employment) twenty-four (24) hours of OPWDD’s Innovations in Employment Supports training.
  • Clean, valid New York State driver’s license
