Implements effective strategies for linking uninsured and underinsured clients to primary care, increases access to appropriate medical care and community-based services, assists individuals in improving health status, and promotes behavior change in using Morton health care services. 

Requirements:

• High School Diploma or GED required.
• · Healthcare, social work, or public health background preferred but not required.
• · Understanding of the healthcare delivery process is required.
• · Excellent interpersonal communication skills are required.
• · Broad knowledge of community resources and services is required.
• · Bilingual (English/Spanish) profiency required.
• Excellent computer skills are required 
•  Required to maintain a high level of confidentiality.
•  · Demonstrated ability to work with community partners, patients, and staff professionally and cooperatively.
•  · Ability to work in a fast-paced environment. 
• · Strong attention to detail. 
• Ability to work independently as well as part of a team.

• Must have and maintain a valid driver’s license.
