Job Description

Registered Dietitian / LDN / RD2Be 

We are seeking a Registered Dietitian Nutritionist (RDN) to provide nutrition care in skilled nursing and behavioral health settings. Responsibilities include nutrition assessments, MDS documentation, high-risk follow-ups, nutrition education, sanitation compliance, and collaboration with interdisciplinary teams to support quality resident care and survey readiness.

  • Lead the Nutrition Care Process
  • Provide nutrition assessments, consultations and care plans
  • Provide nutritional care to patients with complex nutritional needs in rehabilitation settings
  • Provide nutrition expertise, education and consultations to patients and staff
