Registered Dietitian / LDN / RD2Be 

We are seeking a Registered Dietitian Nutritionist (RDN) to provide nutrition care in skilled nursing and behavioral health settings. Responsibilities include nutrition assessments, MDS documentation, high-risk follow-ups, nutrition education, sanitation compliance, and collaboration with interdisciplinary teams to support quality resident care and survey readiness.

Experience with EMR systems such as PCC, Matrix, AHT, Cerner, or Paper Charting preferred. Strong clinical, communication, and organizational skills required.

What You’ll Do

• Lead the Nutrition Care Process
• Provide nutrition assessments, consultations and care plans
• Provide nutritional care to patients with complex nutritional needs in rehabilitation settings
• Collaborating with a team of healthcare professionals including Physicians, Psychiatrists, PAs, Therapists, Occupational Speech Therapists.
• Provide nutrition expertise, education and consultations to patients and staff
