• Practice relationship-based adult medicine with strong operational infrastructure and interdisciplinary support

• 5x8 schedules with strong workflow support

CLINICAL RESPONSIBILITIES
• Provide outpatient adult primary care and chronic disease management
• Manage medically complex patients within a collaborative care environment
• Coordinate care with behavioral health, RN support, specialists, and ancillary services
• Participate in quality initiatives and patient-centered care planning
• Build long-term patient relationships in a mission-driven practice setting

COMPENSATION & BENEFITS
• Base salary: $234,635 – $267,624 annually
• $12,400 annual access bonus paid biweekly for patient panel targets
• Up to $44,000 additional annual productivity bonus potential
• Partnership-track sign-on and retention bonuses up to $100,000 total
• Alternative sign-on structure for non-partnership track physicians
• $10,000 relocation reimbursement
• Additional $10,000 relocation support for qualifying family relocation
• 29 days paid time off including CME, holidays, and sick time
• Comprehensive medical, dental, and vision insurance
• NHSC and federal loan repayment eligibility
• Laptop and cell phone provided
• Language differential pay available for Spanish-speaking providers
