Job Description
Description
At PALM Health, we’re more than a medical and wellness center — we’re a vibrant community dedicated to helping people achieve whole-person, optimal well-being. As a Patient Liaison in our Medical Practice, you’ll be at the heart of that mission.
We’re looking for someone who is caring, collaborative, and committed to service excellence — someone who thrives in a team environment and finds joy in helping others feel supported, informed, and empowered.
What You’ll Do
· Serve as the primary point of contact for a designated medical provider, managing day-to-day coordination and communication.
· Ensure providers are fully prepared for their appointments with organized schedules and up-to-date patient information.
· Deliver exceptional client service by educating patients, managing follow-ups, and facilitating post-visit needs like labs, supplements, and referrals.
· Communicate with warmth, clarity, and attentiveness to ensure seamless continuity of care.
· Coordinate with internal and external providers, retrieve medical records, and help relay wellness recommendations in line with our whole-person approach.
· Support team operations by stepping into clinical or administrative roles when qualified and needed.
Who You Are
· You’re a clear communicator and compassionate listener who understands the power of meaningful connection.
· You anticipate needs, solve problems, and follow through — with a smile.
· You thrive in a collaborative environment and naturally bring others along with your positivity and professionalism.
· You’re passionate about health and wellness and love being part of a team that leads by example.
Requirements
Required Qualifications
· Bachelor’s degree (B.S. or B.A.) or Licensed Practical Nurse (LPN) (required)
· One years’ medical office experience (required); will consider other combinations of education and experience
· Experience with medical terminology and working in a clinic or hospital setting (required)
· Completion of coursework in medical terminology and medical records (preferred)
· Strong computer and word processing skills (required)
· EMR and scheduling experience (preferred)
Why You’ll Love Working at PALM Health
· Join a mission-driven organization where well-being is a way of life.
· Work with experts in medicine, wellness, and hospitality in a beautiful, serene setting.
· Be part of a values-based culture that celebrates warmth, kindness, and professional growth.
· Enjoy comprehensive benefits, access to PALM services, and a community that cares deeply for one another.
