Job Description

Our Hospice is looking for Home Health Aides (HHA) for our home side patients.  This person works with a caseload of patients alongside a RN Case Manager. Our patients and families are in the most important time of their life and need non-skilled nursing to help navigate their pathway. In this position, every day is a little different and you are on the go visiting patients in your community. This role will work Monday-Friday, no holidays or weekends.  The hours are 8 am – 4:30 pm, with some flexibility.

An HHA at Our Hospice Will:

  • Provide routine patient care in the patient’s home or long term care facility per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families.
  • Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet).
  • Observe patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
  • Complete patient records accurately in Home Care Home Base (HCHB) and on a timely basis.
  • Assist in maintaining a clean, healthful environment for the patient.
  • Attend and participates in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
  • Participate in a minimum of twelve (12) hours in-service education per year.
  • Be willing and able to lift patients for routine hygiene as needed

What an HHA at Our Hospice Needs:

  • High school diploma or general education degree (GED)
  • Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana
  • Interest and ability in providing patient care in homes and facilities
  • Ability to travel via your own vehicle 
  • Valid Indiana Driver’s License and car insurance

Salary Range: $20/hr - $21.50/hr plus mileage

Our Hospice Of South Central Indiana

Our Hospice of South Central Indiana is a not-for-profit community-based organization. Since 1980 Our Hospice has served communities in southern Indiana. Three offices located in Columbus, Greensburg, and North Vernon cover a 16 county area. We provide a family-centered team approach to help people with a life-limiting illness manage their symptoms, live fully and keep control over their lives. Our Hospice is certified by Medicare and Medicaid and is a licensed provider of Our Hospice services in the state of Indiana. In January 2004, the new Hospice Center Inpatient Facility located in Columbus, Indiana opened. This center houses the Inpatient Facility and is the administrative hub for the 16-county service area. There are 14 private patient rooms, 2 family rooms, a family dining room, playroom, library, chapel, and gift shop. The exterior grounds allow patients, family members and visitors serene spaces to sit and relax while taking in the beauty of nature. In addition to the 14 patient patios there are a number of public garden areas: the Meditation Garden, the Terrace Garden, and the Serenity Garden. The corporate office for Our Hospice of South Central Indiana, and the Inpatient Facility is located at 2626 East 17th Street in Columbus, Indiana. The following office sites have been developed to provide care to the multi-county service area: Home Services & Inpatient Facility (Columbus), Our Hospice of Decatur/Shelby (Greensburg), and Our Hospice of Jennings County (North Vernon). Our Hospice of South Central Indiana has a very experienced and visionary board that is committed to providing quality care to the terminally ill regardless of their ability to pay.
