Description

The Family Therapy and Support Services Program provides treatment to youth with psychiatric, emotional, and/or behavioral challenges. The target population includes youth adjudicated as Juvenile Delinquent, persons in need of supervision, those at risk of out-of-home placement, and youth in a diversion program to avoid out-of-home placement.

The Caseworker Assistant provides both administrative and hands-on support to the Family Support program by assisting in the provision of case management services to children and families.

Job Responsibilities:

• Provide direct casework services, including emergency intervention services, to clients and their families based on need assessment and treatment plan. 
• Responsible for supervision of family visits and transportation of children and families in the Family Support program. 
• Assist with the maintenance of client files (scanning documents into Agency database, completing reports, performing assessments, etc.). 
• Research and collaborate with community resources to determine eligibility and accessibility of services for clients preparing to transition out of Family support and/or into adulthood.
• Participate as a member of treatment team for each assigned case – responsible for communicating with other team members and the Department of Social Services to ensure integrated service delivery, case continuity, and keep team informed of client progress.

· Bachelor’s Degree.
