Job Description

Position Summary:

Responsible for individualized client services provided in a community/residential setting.  Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate levels/type of services are provided.

Position Description:

  • Withclosesupervision; conducts interviews andadministers assessments and screening instruments
  • Maintain caseloadand productivityby ensuring regular contact with clients tomonitorand assess progress
  • Write service plans, reviewprogressanddetermineupdates needed in collaboration with client
  • Utilize core correctional practiceswhileassistingclients in effective community re-integration
  • Deliveron-going services to clients which mayincludecrisis intervention, advocacy, medication monitoring, transportationassistance, financial budgeting,housingassistance,and referral and linkage to community resources as needed
  • Ensurecompliance with applicable standards and agency policies
  • Document all clinical activities to meet specific program,funderand regulatory criteriatimelyand with accuracy
  • Participate inteammeetingsandassistin making recommendations for client status changes
  • Lead educational groupsand/orfacilitategroup curriculum
  • May require transportation of clients 
  • Adhere to professional standards as outlined by protocols,rulesand regulations
  • Other duties as assigned

Required Knowledge, Skills and Abilities:

Job Requirements:

  • Bachelor's degree in behavioral science, Criminal Justice or related fieldorequivalentwork experience as allowed/required by regulatory bodies
  • May require CDCAor other specialtycertification

  • Positional: Drivingin accordance withjob duties assigned. Frequent standing and walking. Frequentsitting.
  • Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reachingandhandling.
  • Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional useofvisual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
