Job Description

POSITION SUMMARY
Professional member of the home health team who provides skilled nursing visits in the patient’s home under the direction of a plan of treatment established with the physician, with input from the patient/caregiver.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Performs initial and ongoing assessments of client needs using OASIS data elements incorporated into the Comprehensive Assessment.
  • Collaborates with the patient/caregiver and physician to develop and implement the plan of care.
  • Evaluates the effectiveness of the care plan and makes necessary adjustments.
  • Provides for the emotional and physical comfort and safety of the client, considering their rights and cultural background.
  • Receives and transcribes physician orders.
  • Notifies the physician and Agency supervisor of unusual reactions and/or changes in the client’s condition.
  • Documents all observations and treatments in accordance with Agency policies and procedures.
  • Participates in case conferences, team meetings, staff meetings, and Performance Improvement activities as assigned.
  • Provides all prescribed skilled nursing services within scope of training and documented in the plan of care.
  • Prepares clinical progress notes per Agency policies and procedures.
  • Provides 60-day summaries of skilled services and client outcomes to the physician and Agency supervisor.
  • Adheres to all Agency policies, including HIPAA Privacy regulations.
  • Maintains accurate clinical records, notes, and progress reports in accordance with Agency requirements.
  • Participates in patient and family education, as well as in-service programs.

WORK EXPERIENCE

  • One (1) year of field or bedside nursing experience required.

ADDITIONAL REQUIREMENTS

  • This position requires the use of a personal vehicle for travel to patient homes.
  • Proof of valid driver’s license and current auto insurance is required.
  • A clean driving record is required.
