POSITION SUMMARY:

Plan and implement educational activities to ensure patients provide informed consent. Educate clients living with HIV/AIDS on transmission and risk reduction strategies. Collaborate with the case manager, peer navigator and program manager to deliver coordinated care, including medical, educational, social services that address the Social Determinants of Health (SDOH) especially with the LGBTQ+ Community.

DUTIES AND RESPONSIBILITIES:

  1. Design, implement and evaluate Ryan White Supportive Services program’s overall patient education program.
  2. Conducts health intakes and assessments.
  1. Provide group and individual sessions for patients with special health needs or about special health topics, including skill building such as: nutrition, STIs, HTV and AIDS, effective communication with your provider and improving decision-making skills.
  2. Organize, review, and suggest educational materials for ordering and distribution; maintain an inventory of all educational materials.
  3. Provides psychosocial support through individual and group sessions.
  4. Understand the process of Food Voucher and Home Meal Delivery services for program-enrolled clients. Review allowable items during one-on-one sessions with clients and ensure this information is communicated to the Program Manager and Case Manager.
  5. Provides Basic nutrition counseling and support the case manager in the distribution and reconciliation of Food Vouchers / Home meal Deliveries.
  1. Enters data appropriately into charts, medical records, and other systems on a consistent and timely basis.
  1. Assist in the development, distribution, and implementation of educational promotional materials.
  2. Attend and participate in training/informational sessions that are appropriate for Health Educators and in all clinic staff meetings.
  3. Attend community meetings as a representative of the clinic and/or agency as assigned by the Program Manager.
  4. Facilitates related services for in-reach and outreach for health center patients and CHN staff as appropriate.
  5. Other duties as assigned.

Certificate/License required:

  • Associates Degree and/or 2 years of experience in working in a health related field.
