Position Summary:
We are seeking a compassionate and skilled Mental Health Counselor to join our integrated care team. In this role, you will work closely with primary care providers, and other health professionals to deliver evidence-based mental health interventions. This position requires adaptability, a strong commitment to patient care, and a collaborative spirit.

Key Responsibilities:

  • Provide brief, evidence-based therapeutic interventions in a primary care setting.
  • Collaborate with primary care providers to address patients' mental health needs promptly.
  • Conduct comprehensive assessments, including mental health screenings and risk evaluations.
  • Develop and implement individualized treatment plans in coordination with other healthcare providers.
  • Engage in crisis intervention and risk management when necessary.
  • Offer psychoeducation to patients and their families as needed.
  • Document clinical encounters promptly and maintain accurate patient records in compliance with organizational policies.
  • Participate in case conferences, staff meetings, and professional development opportunities.

Compensation: $70,000.00 - $95,775.00 annually

This salary range is based on a 40-hour work week. 

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Qualifications

Qualifications:

  • Master’s degree in Counseling, Psychology, Social Work, or a related field.
  • Current licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or equivalent in NYS. Recent Master’s level graduates (Limited Permit Holders) are considered.
  • Experience working in an integrated or primary care setting is preferred.
  • Strong knowledge of evidence-based therapeutic modalities (e.g., CBT, ACT, brief interventions).
  • Ability to work in a fast-paced, dynamic environment and manage a diverse caseload.
  • Excellent communication and interpersonal skills.
  • Commitment to culturally competent care and the ability to work with diverse populations.
