Job Description

Park Nicollet is looking to hire a Registered Nurse to join our Brookdale Primary Care team!  Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. 

Position Summary:

Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.

Supports multiple areas within a clinic setting with strong interpersonal skill and flexibility while maintaining patient confidentiality and protecting patient rights. Resolves concerns or complaints regarding patient care or service.

We are seeking a Registered Nurse (RN) to join a great, collaborative care team serving a very diverse patient population. This role combines triage and direct patient care to support the care team, address gaps between visits, and improve patient outcomes. The RN will work at the top of their license, using strong clinical judgment to provide timely, patient‑centered care while helping reduce the overall cost of care.

Key Responsibilities

  • Provide triage for patient calls, messages, and walk‑in needs
  • Deliver direct patient care, including assessments, injections, immunizations, and clinical support
  • Identify and address care gaps between visits
  • Collaborate closely with providers and clinic team members
  • Provide culturally responsive patient education and follow‑up
  • Document care accurately in the electronic health record

Work Schedule:

  • Monday- Friday 

  • 7:00am-5:00pm- hours vary 

  • 40 hrs weekly /1.0 FTE

Required Qualifications:

  • Maintain a current Minnesota registered nurse’s licensure. 
  • WI licensure required where applicable.
  • Current BLS provider status in accordance with American Heart Association (AHA) guidelines
  • No online only courses accepted.

Preferred Qualifications:

  • Bachelor’s Degree in Nursing

  • Ambulatory Nursing Experience
  • 1 year of prior experience in nursing preferred
