Job Description

Role: Physical Therapist (PT)
Location: Cheraw, SC-29520
Position: Full Time + Benefits 

Must Have:

+ Graduate of an accredited physical therapy program.
+ Current and unrestricted Physical Therapy license in South Carolina.
+ Current CPR certification.

Responsibilities:

📋 1. Patient Evaluation & Assessment



  • Assess patient mobility, strength, balance, coordination, and endurance
  • Review medical history, physician orders, and diagnostic reports
  • Evaluate pain levels and functional limitations
  • Determine rehabilitation needs and discharge readiness


🚶 2. Mobility & Transfer Training



  • Assist patients with:
    • Bed mobility
    • Sit-to-stand transfers
    • Chair and toilet transfers
    • Ambulation/walking training
  • Train patients to safely use walkers, crutches, or canes


💪 3. Therapeutic Exercise & Rehabilitation



  • Develop individualized exercise programs
  • Improve strength, flexibility, balance, and endurance
  • Provide post-operative rehabilitation (joint replacements, fractures, cardiac surgeries, etc.)
  • Help patients regain functional independence


🏃 4. Early Mobilization



  • Encourage movement as early as medically appropriate
  • Prevent complications from prolonged bed rest such as:
    • Muscle weakness
    • Blood clots
    • Pressure injuries
    • Pneumonia


🧠 5. Neurological & Specialized Rehabilitation



  • Treat patients with:
    • Stroke
    • Spinal cord injuries
    • Brain injuries
    • Neurological disorders
  • Improve coordination, motor control, and balance


⚠️ 6. Fall Prevention & Safety Training



  • Assess fall risks
  • Educate patients and families on safe mobility techniques
  • Recommend safety precautions and assistive devices


🩺 7. Pain Management



  • Use therapeutic techniques and modalities to reduce pain
  • Assist patients in improving movement without increasing discomfort


🤝 8. Interdisciplinary Collaboration



  • Work closely with:
    • Physicians
    • Nurses
    • Occupational Therapists
    • Speech Therapists
    • Case managers
  • Participate in discharge planning meetings


🏠 9. Discharge Planning



  • Assess patient readiness for discharge
  • Recommend:
    • Home health therapy
    • Outpatient therapy
    • Rehabilitation facilities
    • Durable medical equipment (DME)
