Responsible for providing nursing care to patients in their homes .

Essential Functions
Assessing patients' health status and developing individualized care plans.
-Administering medications and treatments as prescribed by the supervising registered nurse or physician.
-Monitoring patients' vital signs and reporting any changes to the supervising nurse or physician. -Providing wound care and dressing changes.
-Performing basic diagnostic tests such as blood pressure checks and glucose monitoring. -Educating patients and their families on their medical conditions and self-care techniques.

Bachelor's Degree Related Field of Study required

Licenses and Credentials
Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only required Licensed Practical Nurse [LPN - State License] - Generic - HR Only required

Experience
Completion of a state-approved practical nursing program and a current LPN license. required

- Basic knowledge of medical terminology and procedures.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and patient demeanor.
- Reliable transportation to travel to patients' homes.
