We are looking for a motivated Physical Therapist to join our integrated medical practice in Fort Wayne, IN. We are looking for a Physical Therapist who is passionate about working closely with patients, creating long term relationships with them while providing high-quality, individualized care. Ideally, the incoming Physical Therapist is a confident team player who enjoys collaborating with an integrated team for high quality, comprehensive patient care.

Duties:

• Patient Consultation-review health and medical histories, exam patients, review pertinent medical records/imaging, and reason for visit
• Consult with referring Physicians/Healthcare team regarding diagnosis as needed
• Develop and implement physical therapy treatment programs based on each patient's particular needs
• Assist patients with all phases of physical therapy treatment and techniques to include exercise instruction, modern modalities, and activities of daily living
• Patient education as to diagnosis, treatment plan options, pre and post treatment care, lifestyle modifications, take-home stretches and exercises, and benefits of continued care
• Documentation and Charting (timely and accurate notes)
• Work cooperatively with the multidisciplinary team and other support staff for comprehensive patient care
• Patient follow up to ensure treatment goals are being met- reassessments

Requirements:

• Graduation from an accredited Physical Therapy Program
• Active Physical Therapy license in IN

Salary (range):

• $90K-$105K
