Job Description

 

Partnership for Children of Essex (PCE) is currently seeking Care Managers!

We are looking for exceptional individuals who are motivated, compassionate, and committed to providing outstanding service each and every day to children and families throughout Essex County.

At PCE, our Care Managers play a critical role in supporting youth and families through a collaborative, strength-based Wraparound approach focused on stability, empowerment, and long-term success.

Key responsibilities include:

  • Utilize a team-based approach in facilitating Child and Family Team meetings
  • Provide face-to-face contact with youth and families in the home and community, in addition to ongoing telephone communication
  • Participate in crisis on-call coverage as needed (PCE provides 24/7 on-call crisis services for all PCE clients)
  • Coordinate and facilitate comprehensive assessments of the strengths and needs of children and families
  • Collaborate in the implementation and development of Individual Service Plans (ISPs) utilizing the Wraparound model of care
  • Coordinate Care Management services across multiple systems and providers
  • Develop and maintain strong community relationships and resource connections for families

Qualifications and Skills:

  • Bachelor’s degree in a related field such as Social Work, Counseling, Psychology, Criminal Justice, or Special Education required; Master’s degree preferred
  • Minimum of one year of related experience in Human Services, Case Management, Social Work, or a similar role
  • Experience with community relations and resource development preferred
  • Prior experience managing Wraparound services is a plus
  • Bilingual candidates (Spanish, Portuguese, and/or Creole) are strongly encouraged to apply
  • Excellent written and verbal communication skills required
  • Must be computer literate
  • Knowledge of the Essex County community preferred
  • Valid driver’s license required
