On Call Coordinator  

General Summary: The On Call Coordinator is responsible for the coordination of patient visits, maintenance, and upkeep of scheduling records as well as accurate and timely communication of scheduling changes between office and field staff. 

Patient Population: N/A 

Essential Functions: 1. Assist in coordinating services provided to Agency patients. 

2. Control and monitor schedule changes for field staff.  

3. Ensure adequate after-hours coverage of patients and staffing. 

4. Bring any scheduling problems to the supervisor’s attention immediately. 

5. Receive referrals, verify insurance eligibility, patient address, phone number, doctor and then staff the case. 

6. Assist in relaying messages to field staff, office staff and account executives. 

7. Perform functions as needed under the direction of the Business Office Manager. 

8. Provide effective communication to clients, their families, team members, and other health care professionals.  

9. Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.

Requirements

Experience:

  • (1) year of experience in general office environment. 
  • Preferred, 1 year of home health care experience.

Skills:

  • Ability to communicate verbally and in writing effectively. 
  • Demonstrates proven decision-making skills. 
  • Must read, write, and comprehend English.

Education:

  • High school diploma or equivalent. 

Licensure/Certification: N/A

Physical Requirements:

  • Prolonged sitting, standing, and walking required.
  • Ability to always handle stressful situations in a calm and courteous manner.
  • Requires working under some stressful conditions to meet deadlines and Company needs.

Environmental/Working Conditions:

  • Works primarily in an office environment.
  • Some exposure to unpleasant weather.
