Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Nurse Practitioner in Portage, Indiana.

POSITION OVERVIEW
• Outpatient Family Practice role serving patients across the lifespan
• Flexible scheduling options including 3x12 schedule opportunities
• Collaborative clinical environment with strong administrative support allowing focus on patient care

• Northwest Indiana location offering convenient regional access and community-centered living

COMPENSATION
• Base Salary: $100,000 – $140,000
• HRSA student loan repayment eligible site
• Relocation assistance available case by case

SCHEDULE
• Flexible outpatient scheduling options
• 3x12 schedule opportunities available
• Outpatient-only clinical setting
• Approximately 2–3 patients per hour

ROLE
• Provide comprehensive outpatient primary care services for patients of all ages
• Manage preventative care, chronic conditions, and acute visits
• Collaborate within team-based care environment to support continuity of care
• Maintain accurate documentation and patient management workflows
• Support mission-focused healthcare delivery within diverse community populations

Requirements

• Board Certified Family Nurse Practitioner REQUIRED
• Comfortable seeing patients across the lifespan REQUIRED

• Commitment to serving diverse community populations REQUIRED
• New graduates and experienced providers welcome
