Job Description
Resources, advocacy and referrals. This position assists patients and families to navigate systems of care utilizing personal, community and governmental resources. The comprehensive impact of health conditions on the client and family will be addressed in partnership with medical providers to meet their various social service concerns.
This position is designed to work with and in support of the Social Services Clinician: MSW, LMSW, LCSW.
