PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the direction and supervision of the Regional Social Work Manager with additional oversight from the Center Director and support from the Social Work Lead (SWL) at the center, the Social Services Coordinator is responsible for providing support to the Social Workers in delivering services to On Lok participants.

DUTIES / RESPONSIBILITIES:

  • Assists the Social Workers in coordinating services for participants.
  • Provides administrative and clerical support to the Social Workers as delegated. Duties will include tasks such as assisting with completion of RCFE and enrollment conference packets, arranging transportation trips, booking appointments in the electronic health record, coordinating activities of daily living, escorting participants, providing assistance with translation and interpretation, assisting with Medi-Cal redetermination and other administrative support matters.
  • Being the participants' advocate, the Social Services Coordinator spends quality time with participants in the day health center and encourages them to participate in conversation and other activities. The Social Services Coordinator will give reports and feedback to the Social Workers on a frequent basis.
  • May attend IDT meetings to provide additional support for participant care, enhancing the quality of care and daily operations as needed.
  • Documents in the electronic health record in a timely and accurate manner.
  • Complies with all On Lok policies & procedures including but not limited to PACE, safety, and confidentiality requirements.
  • Incorporates the Healthcare Hospitality G.R.E.A.T. Practices for 5-Star service.
  • May need to travel within the region as assigned.
  • Other related duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):
1. AA degree, BA preferred, in related field of human services.
2. Minimum one-year experience working with the elderly or frail population required.
3. Ability to work independently and as part of multidisciplinary team.
4. Flexible and organized individual who takes initiative and is very detail-oriented.
5. Computer literate.
6. Bilingual in Spanish and/or Chinese, preferred.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
