The In Home Addiction Counselor / Therapist will complete face to face intake with client/family/caregiver and initiate comprehensive assessment for addiction recovery care. Provide screening and assessment services which shall result in determination of inclusion or exclusion for appropriate program and services. Identify treatment needs, formulate treatment recommendations and develop a comprehensive, assessment-driven treatment plan.
