We are a locally owned and operated home health agency with over 20 years of experience. As a Registered Nurse (RN), you will render 1-on-1 treatment in the comfort of the patient’s home.

Qualifications

  • Graduated with an associate or bachelor’s degree in nursing
  • One (1) year nursing experience in acute care is required
  • One (1) year home health experience is preferred
  • Experience with HCHB EMR is preferred
  • Active RN license in Virginia is required
  • Valid driver’s license

Most seniors prefer to live in the comfort of their own home surrounded by the people, pets and things they hold most dear. We ensure that our clients are safe, comfortable and thriving in their own home by providing the services necessary to make independence at home a reality. We provide skilled nursing, advanced wound care, physical therapy, occupational therapy, speech-language pathology and medical social worker for homebound individuals for treatment and recovery after an accident, surgery, stroke, illness, injury or other disabling event. We also provide Home Health Aides for live-in, overnight & daytime long term senior care in the home. Aide services can include personal hygiene assistance, meal preparation, medication reminders and car transportation to appointments and social activities. Half of all individuals discharged from the hospital require additional care, treatment or rehabilitation. The problem is that care is often not coordinated and it’s divided among many different providers. The client and their family are left bewildered dealing with various people. They just want to get home and feel better. We make the process smoother and less worrisome by integrating all home healthcare services from one company. We call it whole-person care and this approach is only available from us. We are preferred by clients, hospitals and payers with our fully integrated post-acute care solution. We are fully accountable when our client leaves the hospital by managing the care transitions and virtually eliminating unnecessary re-admissions. For most situations, we can provide the same services in the home that are performed in a Skilled Nursing Facility. Home is where you want to be and eliminates facility-acquired infections.
