This includes both in-house and outside appointments. Transportation Coordinator’s will also be responsible for participating in Inter Disciplinary Team (IDT) and Care Planning meetings.    Essential Job Functions   Duties include, but not limited to: Participate as a member of the Inter Disciplinary Team and Care Planning meetings, and report changes in participants’ baseline status to appropriate staff. Collaborate with applicable departments to coordinate transportation for all participants scheduled clinic and outside provider appointments, day center and outings. Involved in the implementation of Quality Improvement activities. Develop and maintain transportation routes manually and with transportation software. Dispatch and communicate new routes to drivers with their participant routes. Prepare transportation metrics and driver performance and safety reporting for Management, as needed. Prepare monthly transportation ride count reporting. Work with Transportation Assistant(s) and Drivers to ensure vehicle maintenance and repairs. Work with other departments to coordinate additional routes, meal delivery, DME and/or care packages. Attend and participate in staff meetings, in-services, projects, and committees as assigned.  May be asked to drive a company vehicle and complete routes to provide support for the department. Understands and navigates the Electronic Health Records system. Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. Accept assigned duties in a cooperative manner; and perform all other related duties as assigned. Be flexible in the schedule of hours worked. May require use of personal vehicle. QualificationsKnowledge, Skills, and Abilities Knowledge of the area in which Innovative Integrated Health is located. Ability to cope with mental and emotional stress related to the position.    Function independently, have flexibility, and personal integrity.  Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.  Effective written and oral communication skills.     Working Conditions and Physical Demands The working conditions and physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling. Ability to communicate verbally and in writing with an excellent comprehension of the English language is required. Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.    Experience Minimum of one (1) year of documented experience in working with a frail or elderly population.
