  Position Summary:   We are currently seeking a Licensed Practical Nurse (LPN) for our Forest Park Health Center location. This full-time role is essential in supporting clinical operations and delivering compassionate, patient-centered care. Apply now to learn more about this role!   Key Responsibilities:
  • Assist providers with direct patient care and clinical procedures.
  • Conduct patient interviews and accurately document medical information in the Electronic Health Record (EHR).
  • Perform routine screenings and tests as ordered by the provider.
  • Handle telephone inquiries, provide clinical advice within scope, and conduct follow-up as needed.
  • Ensure exam rooms are properly stocked, sanitized, and equipped.
  • Coordinate referrals and pre-authorizations, and communicate next steps to patients.
  • Maintain patient confidentiality and comply with all HIPAA regulations.
  Qualifications:
  • Graduate of an accredited Licensed Practical Nursing (LPN) program
  • Current LPN license in the State of Ohio
  • Active CPR certification
  • Preferred: Minimum of one (1) year of LPN experience in a community health, hospital, or private practice setting
  • Strong communication and patient service skills
  • Ability to work effectively in a team-based, fast-paced environment
  • Familiarity with EHR systems is a plus
