Key Responsibilities:

  • Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.
  • Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.
  • Monitor, assess, and document patient progress while ensuring safety and comfort.
  • Educate patients and families on treatment plans, medication management, and discharge planning.
  • Support a culture of excellence, teamwork, and continuous learning.

Qualifications:

  • Required: Current Registered Nurse (RN) license in the State of Connecticut.
  • Required: Graduate of an accredited school of nursing (BSN preferred).
  • Required: Demonstrated competence in the clinical setting.
