Job Description
The Regional Youth Transition Coordinator (R-YTC) will assume primary responsibility for assisting youth within the Transitional Housing/Rapid-Rehousing (TH/RRH) and Mobile Diversion Programs. In the TH/RRH Program, the R-YTC will assist youth in securing and maintaining housing through the use of the Housing First Model. Tasks will include cultivating and maintaining relationships with landlords, linking youth with both transitional and rapid-rehousing options, and providing individualized support for youth throughout their participation in the program. In the Mobile Diversion Program, the YTC will locate homeless youth through conducting street and community outreach, and assist in diverting youth from homelessness and/or linking youth with resources, supports, and housing options. R-YTCs plan and implement outreach efforts within their assigned territory, developing and maintaining positive relationships with relevant community authorities and resources.

Caseloads are small (typically under 10), allowing for more direct hands on work with clients.
Qualifications:
Bachelor's degree with a minimum of one year's experience working in case management.
The ability and desire to work autonomously.
A valid State of Maine driver's license and reliable transportation.
Must be able to pass background checks including DMV, CPS, APS, and Criminal checks.
