Health Information Specialist
Location: Cincinnati, OH

Position Summary

The Health Information Specialist is responsible for managing, safeguarding, and disclosing health records in accordance with HIPAA and organizational policy. This role also provides frontline support to users of the Credible electronic health record (EHR) system, assisting with navigation, troubleshooting, and data quality.

Minimum Qualifications

  • Bachelor’s degree from an accredited institution preferred.
  • Familiarity with HIPAA and FERPA preferred.
  • Experience in medical records, quality assurance, or quality improvement is desirable.
  • Experience with Electronic Health Records; Credible BH experience highly desirable.
  • High proficiency in Microsoft Office (Word, Excel, etc.).
  • Strong written and verbal communication skills with the ability to present information clearly.
  • Ability to work independently and collaboratively.
  • Strong attention to detail with the ability to manage multiple priorities and produce accurate, high‑quality work.

Primary Responsibilities

  • Process, track, and fulfill requests for medical records and protected health information (PHI) from clients, attorneys, insurance companies, and other authorized parties in accordance with HIPAA and organizational policies.
  • Review and verify authorization and legal requirements to determine appropriate disclosure of PHI, ensuring minimum‑necessary standards are met.
  • Accurately document all disclosures within the EHR and maintain required logs and tracking documentation.
  • Prepare, generate, and distribute invoices related to medical record requests, including billing to insurance companies and legal entities.
  • Scan, index, and maintain clinical and administrative documentation in the EHR to ensure completeness, timeliness, and retrievability.
  • Serve as a frontline Credible Help Desk resource by responding to user inquiries, troubleshooting issues, and submitting or resolving task tickets.
  • Assist staff with navigation, data entry, corrections, and general EHR use, following established workflows and guidance materials.
  • Perform routine and targeted audits of EHR data for completeness, accuracy, and compliance with documentation and regulatory standards.
  • Identify documentation errors, omissions, or trends during audits and communicate findings to appropriate stakeholders.
  • Support data quality and compliance initiatives by adhering to records management, retention, and confidentiality policies.
  • Maintain professional communication with internal staff and external requestors to ensure timely resolution of records and EHR‑related requests.
  • Perform other duties as assigned.


Work Environment:

Exposed to a combination of office, school and mental health treatment environments. Exposed to children with behavior problems. May be exposed to shouting and children who are upset.
