Responsibilities:

  • Identifies individual needs through in-person contact with individuals, families, guardians, and significant others;
  • Assures that individuals understand their rights to confidentiality and the limits of confidentiality;
  • Assures that individuals understand their rights as recipients of mental health services, including informed consent and risks and benefits of treatment;
  • Engages in case management activities with individuals which include: intake, assessment, service plan development, linking and coordination of services, service advocacy, and monitoring and evaluation services;
  • Assists individuals in Individual Support Planning process and identification of unmet needs;
  • Participates in crisis intervention services with individuals;
  • Provides supportive counseling or problem-solving activities to assist individuals in developing and maintaining the supports necessary to manage their illness;
  • Completes and ensures the maintenance of all required clinical and administrative documentation;
  • Represents the agency in coordinating service plans with other community providers;
  • Participates in, and leads as necessary, individual-centered interdisciplinary treatment teams;
  • Schedules and leads case reviews according to clinical policies;
  • Participates in a unit team and provides team coverage.

Requirements

Talents, skills, and abilities:

  • Relevant AA/BS Nursing Degree - - including license - - one year relevant work experience, one year supervisory and management experience, knowledge of psychiatric nursing service models, valid Montana Driver’s License, clean driving record, excellent verbal and written communication skills, general skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships with clients, other staff, and the community.
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Relevant AA/BS Nursing Degree - - including Montana State RN License. In addition - and as a condition of continued employment - AWARE's PACT Team Lead must, in good faith, work towards securing the specific professional license and/or professional certification as required by state, federal governing boards and other such certification boards
