Job Description

The Withdrawal Management (WM) Case Manager is responsible for reducing high utilization of withdrawal management and other community services and increasing client engagement by providing individual and group psychotherapy as well as appropriate case management and/or care coordination to clients with substance use disorders and co-occurring disorders, ages 18 and older.

Education, Knowledge, Skills & Experience Required:

  • Bachelor’s degree in related field, preferred
  • CAS or LAC preferred
  • LPC, LMFT or LCSW preferred
  • One or more years related experience and/or training required
  • Bilingual (English/Spanish) preferred
  • Skills or experience in integrated health approaches preferred

Essential Duties:

  • This role may require cross functional responsibilities and contingency support including but not limited to: Adaptability to staffing needs: In the event of a staffing crisis or emergency, the employee may need to be deployed to another unit / team, whether working with adults or youth, within Jefferson Center for which they are qualified and credentialed, as determined by leadership.
  • Skill Utilization: Staff may be assigned to support different teams or departments to address client needs while remaining within the scope of their professional licensure and credentialing.
  • Provide case/care coordination and other brief services for EC, IC, high utilizer clients, and other consumers as needed.
  • Works constructively with consumers to reach agreed upon outcomes, and coordinates care with internal and external providers.
  • Empower clients to access identified resources and assists with navigating systems to meet identified outcomes.
  • Assist with transportation to appointments or treatment placement, as needed.
  • Provides individual, group and family treatment as determined by the Manager.
  • Identifies risk factors including lethality for suicidal, homicidal and/or grave disability. As appropriate, complete involuntary mental health holds (27-65) according to Center protocol.
  • Provides behavioral and diagnostic assessments and initial engagement strategies, treatment and discharge planning, and case management to the withdrawal management consumers. Provides level of care assessment and referral.
  • Assists line staff with providing guidance, safety, stability and direction to clients, including de-escalating potentially dangerous situations.
  • Builds and maintains relationships with other providers to facilitate an efficient referral process.
  • Works to develop effective ways to research, update and communicate Jefferson Center and community resources to clients.
  • Assist in data collection and tracking for monthly reports regarding utilization of services and connections to treatment.
  • Adhere to all Withdrawal Management and Jefferson Center Policies and Procedures.
  • Participate in supervision by coming prepared with an agenda. Report high risk/problem cases, utilize a problem-solving approach, and work to integrate feedback.
  • Perform accurate and timely documentation at all times.
  • Adhere with Corporate Compliance requirements in accordance with regulatory requirements and clinical guidelines.
  • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
  • Maintain effective interpersonal relations with consumers, peers, upper management, visitors, and the general public. Use language and behavior to promote dignity and respect.
  • Demonstrate cultural competence in working with clients from diverse backgrounds, and exhibit knowledge and skills to develop therapeutic alliance with patients.
  • Participate in staff development activities to enhance professional growth.
  • Ability to prioritize responsibilities and effectively manage time independently.
  • Address the whole health needs of the client by ensuring that appropriate releases are in place for other providers, and by making appropriate referrals as needed.
  • Understand how trauma impacts the lives of the people being served, ensure that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization.
  • Ensure that delivery practices are guided by the principles of trauma informed care and the principles of addiction treatment.

Salary Grade 49 (High School/GED): $50,000 annually ($24.04/hr)*

Salary Grade 50 (Bachelor's Degree): $55,000 annually ($26.45/hr)*

Additional Salary Information*:

  • The annual salary listed above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
