Job Description

Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Schedule: 8:30am-5:00pm; M-F

Position Summary:

Under direction of the PACE Nursing Staff, the Certified Nursing Assistant (CNA) is responsible for the delivery of personal care and related services to PACE participants in collaboration with the Interdisciplinary Team (IDT) and in accordance with the plan of care (POC).  Assigned participants are primarily in the clinic/day center but may also be in other settings such as a home, long term care facility, assisted living facility or hospital. 

Position Qualifications:

  • High school diploma preferred.
  • State Registered Nurse Aide (SRNA) certification required in the state of Kentucky.
  • One year home health aide or nursing assistant experience preferred.
  • Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
  • Use of technology-based tools, including electronic medical records, preferred. 
  • Must maintain CPR certification.
  • Must have a valid driver’s license in accordance to policy with proof of automobile insurance required.
  • Knowledge of safety and infection control requirements for healthcare facilities.
  • Knowledge of medical equipment use and maintenance.
  • Ability to communicate clearly and effectively.
