Summary:

The Program Intake Coordinator is responsible for the overall fill rate of CDS Monarch programs (Day Habilitation, Recreation/Respite, Community Prevocational Programs, etc.). Screens all applications for all services and processes all applications for CDS Monarch (ensures applications contain all information). The Day Services Intake Coordinator chairs the Intake Committee for the agency, controls membership of the committee, and ensures all applications are processed through the committee. Works with Care Managers, DDRO Representatives, and other agencies to market CDS Monarch programs.

Essential Job Functions:

•    Screen applications for all services and contacts Care Managers to set up Intake Committee meetings.

•    Maintains waitlists for all services in the agency intake folder.

•    Completes written summary for each individual’s and presents to Intake Committee.

•    Chairs the Agency Intake Committee, maintains the required membership for the committee.

•    Completes DDP1, DDP2’s, Participant Change Form, Authorization request form for Day Hab. And program admissions, transfers and discharges. Notifies Business Office of billing start date.

•    Completes pre-admission paperwork with families prior to admission into a Day Program.

•    Works with Program Managers to fill Day Hab. vacancies.

•    Completes/Coordinates tours at all programs for applicants that are interested in CDS Monarch services.

•    Ensure that each program maintains its required fill rate.

•    Coordinates marketing for CDS Monarch to include open houses, written communication, in person events, social media use, and other avenues as appropriate.

•    Responsible for writing and updating intake/discharge policies and procedures.

•    Perform all other duties relevant to the position as assigned by supervisor.

Essential Academic and Experience Requirements 

•    Bachelor’s degree in Education, Social Work, Psychology, or Human Services related field with at least one-year experience with developmental or intellectual disabilities, or an equivalent combination of education and experience preferred.

•    Previous experience with intake not required but preferred.

•    Must possess a valid NYS driver’s license and meet CDS Monarch Vehicle Operator Requirements.

•    Ability to work independently and motivate others.

•    Ability to communicate effectively, both oral and in writing.

•    Maintain all required certifications/training by State regulations and CDS Monarch policy.

All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Executive Vice President, in collaboration with Human Resources.

•    Attend mandatory education and training modules as scheduled; obtain and maintain required certifications.

•    Maintain all required certifications/training by State regulations and CDS policy

The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
