Registered Nurse RN Case Manager Home Health
Honesdale, PA (Wayne County, Hawley, Carbondale Area)
Interim HealthCare of Scranton
$38–$40/hour + $5,000 Sign-On Bonus + Weekly Pay

Immediate opening for a Registered Nurse RN Case Manager Home Health in Honesdale, PA and surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.

PAY AND BENEFITS

  • $38–$40 per hour
  • $5,000 sign-on bonus

SCHEDULE

  • Monday through Friday 8:30 AM to 5:00 PM
  • On-call, weekend, and holiday rotation

REGISTERED NURSE RN CASE MANAGER RESPONSIBILITIES

  • Manage a home health caseload in Honesdale and surrounding areas
  • Perform RN assessments and reassessments
  • Develop and update care plans
  • Coordinate care with physicians and healthcare team
  • Complete home health documentation and ensure compliance
  • Educate patients and families

REQUIREMENTS

  • Active Pennsylvania Registered Nurse RN license
  • Minimum 1 year home health RN experience
  • Case Manager or Case Management experience preferred
  • CPR certification
  • Valid driver’s license and reliable transportation
