Job Summary

Pilsen Wellness Center is seeking a Mental Health Professional (MHP) to join our PATH (Projects for Assistance in Transition from Homelessness) team, funded by SAMHSA. PATH is a federally funded initiative, designed to connect individuals with SMI who are experiencing homelessness to mental health, substance use, housing, and support services. Services may include outreach, screening, treatment referrals, and housing assistance. The ideal candidate will work with individuals experiencing homelessness and living with Serious Mental Illness (SMI)—including schizophrenia, bipolar disorder, and major depression.

Responsibilities

  • Provides crisis intervention and de-escalation support as needed
  • Conducts ongoing assessment and therapeutic engagement
  • Maintain timely and accurate clinical documentation
  • Assists with data collection and reporting requirements
  • Coordinate care with internal and external resources
  • Adhere to ethical and confidentiality standards

Qualifications

  • Bachelor's degree in Counseling, Social Work, Psychology, or related field required
  • Active Drivers License
  • Bilingual (English/Spanish) preferred
  • Community mental health experience preferred
  • Experience with field-based outreach and engagement
  • Skills in crisis de-escalation, harm reduction, and Housing First principles
  • Cultural humility and the ability to build trust with diverse populations
  • Knowledge of mental health systems, housing resources, and public benefits
  • Flexibility to work in shelters, encampments, and street settings
  • Strong documentation and collaboration skills
