Physical Therapist - Home Health

Minimum Qualifications:

  • Must be licensed as a Physical Therapist in the state of Oregon
  • Graduate from an accredited program with a degree in Physical Therapy
  • CPR Certification
  • National Provider Identifier (NPI) required upon hire

Preferred Qualifications:

  • Experience in Home Health
  • Organizational skills to facilitate program development
  • Strives to positively represent their discipline and AgeRight Home Care in the community

Responsibilities:

  • Complete assessments for every patient upon admission
  • Establish treatment plans
  • Teaches patient and family/caregivers in meeting individual treatment goals, and treatment plan implementation
