Description

Come join Psychiatric Medical Care's adolescent behavioral health outpatient program, Embrace U. Embrace U is a combined IOP and PHP treating patients ages 10-18. 

Location: Brentwood, TN- Onsite

Salary:  $55,000-$60,000 dependent upon licensure 

Patient Population: Adolescents ages 10-18

Schedule:  Mon-Fri 10am-6pm during the school year, 8am-4:30pm during the summer

Job purpose

  • The adolescent care coordinator provides quality administrative and clerical services for program staff and assists Embrace U clients with care needs. They also serve as back up coverage to other Embrace U staff. The adolescent care coordinator’s responsibilities include assisting with the inquiry and intake process, providing clerical support to team members, assisting with discharge planning, working with clients during PHP time, holding psychoeducational client groups, and assisting with client care.

A. Care: All employees are expected to provide the best possible client care.

  1. Ensure client care needs are met while at the program.
  2. Ensure completion of the Census report as directed.
  3. Assist clients with completion of consents and client admission process as directed.
  4. Take client vital signs as directed.
  5. Participate in treatment team meetings and coordination of care when needed.
  6. Coordinate snacks for clients.
  7. Complete medical records including filing and form tracking.
  8. Responsible for inventory of supplies.
  9. Responsible for refrigerator logs and necessary action steps for temperature variances.
  10. Monitor clients during PHP time.
  11. Perform group during IOP time.
  12. Any other tasks as assigned by supervisor.
  13. Assist the NP with intakes and follow ups as needed.

B. Compliance: Ensure the program operates within all regulations.

  1. Comply with all regulations.
  2. Perform billing functions required for the program as directed.
  3. Ensure client insurance verification and concurrent reviews are completed.

C. Community: Become a wonderful addition to your local community.

  1. Demonstrate an understanding of community education definitions and activities.
  2. Participate in customer service training as it relates to community integration
  3. Keep up to date community education contacts as directed.
  4. Participate in cohesive team environment to ensure program success.

Working conditions

  • Adolescent Care Coordinator will be exposed to virus, disease, infection from clients.

Direct reports

  • This position has no direct reports.

Requirements

  • Education: Graduation from a state approved school of nursing.
  • Licensure: Licensed Practical Nurse or Registered Nurse in the State of practice.
  • Experience: Psychiatric Nursing experience preferred.
  • Other skills: Patient and family teaching; teaching/learning materials development; problem solving; complaint resolution; ability to make presentations to outside groups, good problem solver, results oriented.

Physical requirements 

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position required intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting, driving and supporting clients.
