Role Summary: Carries out the services of the programs by assisting participants in obtaining and maintaining competitive employment that is consistent with their vocational goals. Engages participants and establishes trusting, collaborative relationships directed toward a goal of competitive employment in community job settings with other workers. This position is being offered in Albuquerque. This is a full-time position 40 hours a week, Monday through Friday. This is not a remote position. Position requires regular travel to various locations daily to provide job coaching and development. Must have reliable transportation.

Principal Accountabilities: (essential job functions):

  1. Monitor and observe LifeROOTS’ Participants in employment/work settings.
  2. Completes required paperwork accurately and timely according to departmental procedures.
  3. Communicates effectively with supervisor, LifeROOTS Service Coordinators, employer representatives, and other relevant parties.
  4. Participates in the development of the participant's Individual Service Plan (ISP).
  5. Participates in job development activities in coordination w/Employment Services Manager.
  6. Participates in departmental projects.
  7. Presents program in a positive manner to the public.
  8. Flexible in responding to varied job assignments - nights, weekends and holiday work may be required.
  9. Teach individual job skills, requirement utilizing job task analysis, job specific supports including assistive technology.
  10. Provide personal assistance, as needed, and able to according to limitations/doctor’s restrictions.
  11. Build natural support network between the individual and co-workers.
  12. Identify/solve problems before they become a crisis for the individual, employer and co- workers.
  13. Assists the individual side by side at the job site.
  14. Analyzes job requirements and break into manageable components that can be taught to the individual.
  15. Though the main duties of this position will be Job Coaching, this position must cross train with the Community Integration Coach (CIC’s) Job Description.
  16. Attend all meetings unless prior arrangements have been made with the department head.
  17. Complies with DD Waiver, Mi Via and State General Funds Standards.
  18. Maintains conduct that is professional and ethical at all times.
  19. Complies with all applicable LifeROOTS policies and procedures.
  20. Relies on instructions and pre-established guidelines to perform the functions of the job. Primary job functions do not typically require exercising independent judgment.
  21. Responsible for participation in the company’s quality improvement program and ensures that department applicable quality improvement requirements are fulfilled.
  22. Responsible for participation in the company’s financial management and ensures that all department-applicable fiscal responsibilities are fulfilled.
  23. Responsible for participation in the company’s Health and Safety program and ensure that all department-applicable safety requirements are fulfilled.
  24. Performs other related duties as assigned by the Employment Services Manager.

Requirements

You’ll support participants in both short-term and long-term community-based jobs. You’ll work with employers to customize roles, using creative solutions like job carving to ensure the best fit for each individual. You’ll provide on-the-job training and coaching to help participants build confidence and succeed in their roles. You’ll promote workplace inclusion, encourage open communication, and support self-advocacy. You’ll assist with workplace accommodations, introduce assistive technology, and help navigate ADA-related needs. You’ll maintain clear and accurate documentation, including ISPs and IEE’s. You’ll use local employment resources to help individuals achieve their career goals. Familiarity with DD waiver is a plus.

The knowledge and experience needed for this position are: 

You’re organized, enjoy working with people, and are passionate about creating inclusive workplaces and empowering others. You’re comfortable providing personal care support when needed, detail-oriented, and able to maintain accurate documentation in compliance with regulations. You have excellent follow-through, work well independently, and are proficient in Microsoft Office, typing 50+ words per minute. The ideal candidate brings a collaborative spirit, strong communication skills, a caring and passionate attitude, integrity, and a drive to do the right thing while staying fully engaged and hard-working.

 Conditions of Employment:

  1. HS Diploma or GED.
  2. One year of experience working with people with disabilities.
  3. Experience working in business and industry preferable.
  4. Participation in recent continuing education programs related to Community Services, per licensing requirements.
  5. Must complete required training within 180 days of employment.
  6. Fingerprints, background check, and drug screen.
  7. Valid/current driver’s license with good driving record and must have current auto insurance as required by applicable state law.
  8. Routinely operates a company vehicle or a personal vehicle while on company business.
  9. Must be at least 18 years of age.
  10. Regular and consistent use of keyboard and mouse.
  11. Physically able to perform essential functions of position without hazard to themselves or others.
  12. The work is non-sedentary, requiring walking, assisting in lifting and transferring participants, bending, and lifting carrying 50 pounds.
  13. Exposure to changing weather conditions.
  14. Must have the capacity to toilet and provide personal care with people with disabilities.
  15. Assists with wheelchair transfer of non-ambulatory participants.
