Job Description
Job Title: Case Manager
Team: Housing
Reports To: Director of Housing
Hours: 40 hours per week
Compensation: Starting pay $24-26/hr depending on experience
Benefits: 100% paid medical and dental insurance, AD&D, short and long term disability insurance, holidays, and paid leave

The Care Navigator helps clients get the support they need to access a wide range of resources. Care Navigator provide personalized support and guidance to access HopeSource programs as well as provide a direct connection to vetted community agencies and resources that support the needs of the individual. Supports include but are not limited to addressing economic barriers, providing basic needs (including, but not limited to, housing, food, transportation, utility payments, and material goods), and other supportive services such as workforce development, health and mental health care, recovery, and life skills education.
Care Navigator are skilled, effective communicators who have a problem solving mindset and are responsible for ongoing community resource knowledge. Care Navigator collaborate extensively within the community to overcome logistical barriers and increase care quality, improve care continuity, and improve the customer experience.
