Job Description

Hourly wage: $20.80 ($37,856 a year) 

35 hour work week

Job Duties:

The successful candidate will be expected to:

  • Receive referrals and ensure transitions process is followed through for each individual.
  • Provide information and assistance for individuals and families who are considering or are in the process of transitioning from an institution to living in the community.
  • Coordinate visits to institutions, provide potential participants with unbiased information regarding available home and community-based services, participate in discharge planning, and coordinate with other relevant social services agencies involved in the discharge and transition care of the individual. 
  • Collaborate with New York Association on Independent Living (NYAIL) staff to determine and resolve barriers to transition. 
  • Explain “peer mentor” and “good neighbor” programs to potential participants and if appropriate provide referral to these programs. 
  • Ensure all significant individual data and service provision details are current and accurate and that they are documented in a timely manner into the New York Association on Independent Living (NYAIL) database. 

Education:

  • High School Diploma or equivalent required. College degree preferred. 

Knowledge, Skills & Abilities:

  • Nursing Home Transition and Diversion/Traumatic Brain Injury, Waiver, Office of People with Developmental Disabilities system, various disabilities and how they impact individuals’ ability to function, strongly preferred. 
  • Entitlement programs, American with Disabilities Act, knowledge of issues faced by persons of all ages who are in need to long-term services and a commitment to person-centered planning and individual choice preferred. 
  • Strong advocacy, planning, community networking, innovation, collaboration, flexibility, organization, written and oral communication skills as well as basic responsiveness to working with a diverse population. 
  • Valid driver’s license and dependable transportation is required. 
  • New York Association on all Independent Living, department and agency training required initially, annually, and ongoing. All department of Health background and fingerprinting requirements must be met.
