We are searching for Master’s-prepared Clinical Case Manager for our Social Services Inpatient Team.
What Role Will You Play?
Social Services Clinical Case Managers at Fairfax are part of an interdisciplinary treatment team charged with providing quality clinical case management services to our patients from admission to discharge. Clinical Case Managers are empowered to assess their patients’ needs, to collaborate with patients to create safe and appropriate discharge plans, to participate in diagnostics and treatment planning, and to work with our patients’ family/support/outpatient team for coordination of the discharge plan. A bachelors or master’s degree from an accredited college or university in a mental/behavioral health field is required (an MSW is preferred) as well as the desire to help our patients improve their lives. You will also need to obtain an Agency Affiliated Counselor credential unless you have a clinical license. All Fairfax employees must successfully pass the Handle With Care course completed during New Employee Orientation. A positive attitude and outlook will make you successful in this role, as well as an understanding of the importance of regulatory-driven deadlines and the ability to stay organized while needing to pivot based on competing important priorities.
