Position Summary
The Supports Coordinator - Adults serves as the primary advocate and resource broker for individuals with intellectual and developmental disabilities. This role ensures access to community resources, entitlements, and supports that promote independence, self-determination, and quality of life. Responsibilities include developing and implementing Individual Plans of Service (IPOS), coordinating care, and fostering community connections.
