HOME COORDINATOR - Team Leader

QUALIFICATIONS

  • The HC must be at least 18 years of age, high school graduate/GED
  • Reliability, kindness, compassion, patience, and a positive role model
  • Good communication skills
  • Basic financial math skills
  • 1-year Full Time equivalent work experience in service delivery to individuals with intellectual and developmental disabilities. (May be substituted with service delivery to individuals in a hospital or nursing home setting.)
  • Driver’s License, car insurance and reliable transportation for every shift is required (mileage, for client transportation, is reimbursed at 62.5 cents per mile)

CORE RESPONSIBLITIES

  • Assist clients with intellectual and developmental disabilities to complete daily life activities including personal care, household chores, shopping, and recreational and social activities.
  • Provide companionship and emotional support to each individual served
  • Train and mentor new and existing staff
  • Complete daily activities and support job delegation of daily activities to staff members
  • Assure all client's doctors’ appointments are scheduled and attended
  • Assure all information in the client's books are kept updated and inform the Program Manager of any changes in the client's life
  • Monitor client's spending and maintain accurate ledgers ensuring all bills are paid in a timely fashion

Our support areas include skill development, recreation, social skills, medical care, transportation, and financial assistance. • INDIVIDUALIZED SKILLS DEVELOPMENT/COMMUNITY INTEGRATION - Provides services for clients not yet living independently, helping them develop life skills and build community connections. • REACHING INDEPENDENCE THROUGH SOCIAL ENGAGEMENT (RISE) Day Program- Community-focused classes designed by participants and qualified staff emphasize social, educational, and physical activities. • PROJECT HEART - Evening classes focused on health and safety may include topics such as cooking, nutrition, exercise, personal hygiene, driver’s education, healthy relationships, and internet security.
