CCM Health

CCM Health offers an unprecedented advance in healthcare by using its proprietary telemedicine technology and highly-trained team of experienced healthcare coordinators to manage patient's health and reduce healthcare costs. CCM Health has created software technologies that employ machine learning to feed our system’s Artificial Intelligence. The system considers hundreds of conditions and markers of patients’ disease states and medical histories to assign a risk category, alert the case manager in an attempt to prevent hospitalization, and guide the user in a preapproved clinical plan of action. This allows us to standardize the quality of care and allows us to hire, train, and implement medical staff in a “fast track” model. We are able to modify the clinical protocol and ensure immediate adoption with the goal of achieving desired clinical outcomes.
