A Day in the Life as a LPN Visit Nurse with our Home Health team: 

  • Provide hands‑on nursing care guided by individualized care plans, ensuring each client receives the support they need to remain safe and independent at home.
  • Monitor changes in health and well‑being, promptly reporting updates to the care team and ensuring continuity and quality of care.
  • Document your visits in the electronic medical record (EMR) with accuracy and clarity, helping the entire team stay connected and informed.
  • You’ll participate in on‑call rotations, including evenings, weekends, and holidays—providing urgent support with calm professionalism when it’s needed most.
  • You’ll join nursing case conferences, attend team meetings, and complete ongoing training, contributing to a collaborative environment that strengthens care quality and professional growth.

Open Shifts Available: Full-Time Monday-Friday with rotating on-call (weekends)
