Job Description

Earn $24.04/hour with full benefits or $26.04/hour with the enhanced pay option + $4,500/yearly mileage stipend
Position Description
The LPN is a vital member of the Assertive Community Treatment (ACT) Team, providing community-based behavioral health support to adults with mental illness. LPNs help ensure members receive coordinated, person-centered and holistic care. This role is field-based, requiring travel within the region to support individuals where they live and receive service.
The LPN provides medication management, medication education, crisis intervention, medication administration to consumers of the ACT Team. The LPN is also responsible for following the orders of the team's prescribers to ensure the members are receiving adequate treatment and interventions. LPNs assist with PCP and other medical specialty coordination to ensure the members needs are being met. The LPN assist with assessment and the development of the person-centered treatment plan. This is community- based work.
