Community of Hope is seeking a Perinatal Care Case Manager, HONEY to assess patient needs during the pregnancy and postpartum period, developing individualized care plans and ensuring patients have timely access to healthcare and educational services that will remove barriers to care and improve overall outcomes for parent and baby.  This is a full-time, temporary position located at our Family Health & Birth Center in NE, Washington, DC

What You'll Do

  • Ensures access to supportive and educational services for prenatal and postnatal patients with an emphasis on participating in prenatal and postpartum healthcare (whether at Community of Hope or another prenatal provider), increasing understanding of maternal and child health needs, and coordinating social services and parenting resources.
  • Maintains a caseload of approximately 30 participants referred from the Virginia Williams Family Resource Center, DC-based housing providers, and/or healthcare providers. 
  • Conducts thorough assessment on all prenatal/postpartum clients utilizing Healthy Screening tools (HITS, 4P’s, PHQ-9, Edinburgh). Follows up in accordance with the HONEY Crisis Protocol and refers clients appropriately based on risk.
  • Develops individual care plans with designated periods with particular emphasis on risk factors, including but not limited to social needs, barriers to care, pregnancy planning, and food insecurity.  Tracks progress on individual care plans developed with patients to achieve their self-identified health goals.  
  • Conducts client outreach through telephone calls, text, emails, and drop-in visits as needed, including collaboration with housing case manager to maintain engagement and address any barriers to program participation.
  • Utilizes the HONEY Care Passport to provide health education to clients in a variety of settings, with an emphasis of increasing understanding of healthy pregnancy, self- management of care and health systems.
  • Assists as a liaison between the client and their homeless services provider, clinical staff, and other health care professionals.
  • Discharges clients in accordance with HONEY protocols, including exits at 6 months postpartum, non-responsiveness, or client opt-out. Ensures clients are connected to all necessary ongoing supports, such as mental health services, home visiting programs, and family resource centers upon exit from the program. 
  • Documents any relevant resource connections and/or referrals, client encounters, including face-to-face, telephone, and electronic communications, in eCW within 2-3 business days and other systems per HIPAA and departmental policies and procedures.
  • Completes other tasks as requested by supervisor.

Requirements

Must-Haves

  • High school diploma, G.E.D, or equivalent experience. 
  • Valid BLS Certificate.  
  • Experience providing social services, care coordination or health care education.
  • Familiarity with perinatal services and standard of care.
  • Strong organizational skills. Ability to multi-task and problem-solve.
  • Strong written and verbal communication skills.
  • Ability to travel between sites required.
  • Willingness and ability to make home visits and hospital visits, and work occasional evenings and weekends.
  • Proof of required vaccinations is required.  This includes, but may not be limited to, Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.

Nice-to-Haves

  • B.A./B.S. or equivalent experience required; B.A./B.S. in social work, education, or related field.
  • Experience working with an under-resource population, including pregnant people.
  • Familiarity with the Medical Assistance system and community resources strongly.
