About the Opportunity

Stepping Stone Behavioral Health is seeking dedicated and compassionateLicensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC)Ā to join ourĀ outpatient behavioral health team.
ThisĀ mobile clinician roleĀ provides mental health and substance use counselingĀ directly inside skilled nursing and long-term care facilities, offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges.

What You’ll Do

  • ConductĀ bio-psychosocial assessmentsĀ and develop individualized treatment plans.

  • ProvideĀ individual and group counseling, teaching coping and recovery skills.

  • Identify resident needs, set measurable goals, and guide them toward emotional stability.

  • Participate inĀ treatment team meetingsĀ and collaborate with facility staff and community partners.

  • Maintain timely and accurateĀ clinical documentationĀ in EMR systems.

  • Assist withĀ telehealth sessionsĀ and submit required daily reports.

  • Facilitate safe discharge planning and coordinate aftercare services.

  • Uphold confidentiality, professionalism, and compliance with facility and state regulations.

Requirements

  • Must beĀ 21 years or olderĀ with aĀ valid driver’s license.

  • ActiveĀ Ohio license: LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.

  • Ohio Medicaid Billing NumberĀ andĀ NPI NumberĀ required (or ability to obtain).

  • Minimum of anĀ Associate’s DegreeĀ (Bachelor’s or Master’s preferred).

  • Strong interpersonal, organizational, and documentation skills.
